From Fear to Hope – One Patient’s Journey

When “Manuel” first came to Community Care Clinic, he was in crisis.
His diabetes was dangerously uncontrolled, with an A1C level greater than 15.5. He appeared weak and frail, struggling to rise from a chair during his appointment. Beyond the physical symptoms, there was also fear — fear of what might happen if his health continued to decline.
But at Community Care Clinic, no patient walks through our doors alone.
Our care team immediately began working alongside him to create a path forward. He started diabetes medication and received personalized education and encouragement from our Diabetes Educator to help him better understand how to manage his condition day-to-day. At the same time, he began physical therapy to rebuild his strength and mobility.
Little by little, progress came.
At a recent follow-up appointment, our providers shared remarkable news: his A1C had dropped to 7.8.
He and his wife both began to cry — overwhelmed by gratitude, relief, and hope for the future.
What makes this story even more meaningful is that the changes extend beyond lab numbers. After just a few physical therapy sessions, he is already moving more easily and gaining strength. Our team is also helping coordinate specialty care through Atrium to address his cataracts so he can continue improving his quality of life.
